Short and Medium-Term Trainees
UCLA Physicians
If you are a UCLA Physician, please complete the UCLA Physician Interest Form below. This training program does not follow the academic year and therefore does not have an application deadline
UCLA Undergraduates
If you are a UCLA Undergraduate student, please complete the application below by April 30, 2026 at 11:59 PM and mark "Undergraduate". Undergraduate trainees do not need to submit letters of recommendation and will not go through the interview process. Applications will be reviewed and decisions will be sent via email by June 1st.
Long-Term Trainees

Graduate and Practitioner Trainees
To apply to be a UC-LEND trainee, you will need to submit the following at the application link below:
- Contact information
- Response to two short-answer questions (250 words maximum):
- What interests you about the UC-LEND program and what do you hope to achieve as a UC-LEND trainee?
- How do you define leadership?
- Current Resume or Curriculum vitae
- Graduate students only: Current unofficial UCLA or UC Riverside transcript
- 2 Letters of Recommendation (see below)
Letters of Recommendation
Long-term trainee applicants are also expected to have two completed UC-LEND Recommendation Forms submitted by advisors, supervisors, or other individuals that can speak to the applicant’s ability to succeed in a professional and/or academic environment.
- At least one recommendation must be from either a faculty advisor or supervisor (UC-LEND faculty may serve as your reference)
- Recommenders should send forms to uc-lend@mednet.ucla.edu directly. Applicants should not submit forms on the recommender’s behalf.

Please submit all materials except recommendation forms at the UC-LEND application link below by April 30th, 2026.
Once a completed application has been submitted and recommendations have been received, applicants will be contacted to schedule an interview. For more information or questions about applications, please contact uc-lend@mednet.ucla.edu.
This training program is funded by the Health Resources and Services Administration (HRSA) under training grant T73MC30114. Stipends are contingent upon the renewal of the federally funded UC-LEND Training Program grant. If stipends are available, eligibility for the stipend includes acceptance into UC-LEND as a long-term trainee and status as a U.S. Citizen, non-citizen national, or other permanent resident of the United States.
